COVID-19 Update :  

We are now "Open". Looking Forward To See you!!

Our office reopened on Jun 1st as per guidelines and specifications from CDC, ADA and CSDA. You will notice some changes in our check in process as we are accommodating these guidelines for the safety of our patients and staff. Thanks a lot for your cooperation.

Pre-Appointment Screening Process :

You will receive an email or phone call from our office with a COVID-19 pre screening questionnaire . Please fill this questionnaire and send it back to us with in 24 hours of your scheduled appointment. You can also download this form from our website or following link.

On the Day of Appointment:

When you arrive for appointment, please stay in your car and inform our office. You will receive a text or phone call from the office when treatment room is ready for you. We ask companions and parents to stay in the vehicle as our reception area will be closed for the time being

In-Office Patient Registration Procedures:

We ask you to enter the office with mask and use hand sanitizer which will be placed in the office reception as well as other areas. We will guide you through the whole process. Your temperature will be taken when you arrive at the office for appointment and the questions on the screening form will be repeated in order to ensure nothing has changed since you have submitted the form.
